FRA SEEKS PROPOSALS FOR THE FOLLOWING:





1.00 Fermilab housing office Linen contract





Your written response to this solicitation must be in accordance with the following additional requirements:

The following terms and conditions shall become a part of any subsequent purchase order/subcontract resulting from this solicitation:

Documents to be returned with proposal submittal:



1. RFP form 340983-MTC - highlighted sections completed



2. Fermilab Housing Office linen contract bid sheet - completed in entirety



3. SARC - Subcontractor Annual Reps and Certs (Rev. 3) – all applicable sections completed and form signed.



4. PUR-466 (Rev 1.) – Proposal Certification – all applicable sections completed and form signed.



5. Exhibit A - Attachment A completed/signed



6. A separate document, including any/all exceptions to FRA LLC General Terms and Conditions for Services (06.2022). Exceptions to FRA Terms and Conditions shall be presented with RFP submittal.



7. Valid certificate of insurance* *Required upon award
